Hi All! I stumbled across this group while doing a Cushings search and I must say I am so glad that I did! Everyone here seems amazing, and my heart goes out to all of you and your furbabies. My Frisco was diagnosed about a year and a half ago with cushings. At that time his only symptoms were drinking and peeing more than usual but nothing super excessive (a few accidents in the house), some weight gain, panting, and a good appetite (which he has always had). Tests were done to confirm cushings (I do have all of his records so I will post results at the bottom) but both his vet and I felt the symptoms were so minor that treatment was not necessary at that time.

Frisco did real well up until about 6 months ago, when I noticed he was having a harder time getting up and down stairs (nothing too crazy, just not with ease as usual). He had gained some more weight and had a pot belly. The drinking and peeing came in bouts--for the most part normal, but on occasion I would notice an increase in both that lasted about a week, then it was back to what a normal dog would be. He still LOVED his food, as he always has. He never whined or cried or got aggressive about food though.

Fast forward to today--Frisco is not doing well at all. It breaks my heart. He can barely use his hind legs, so he lays pretty much in one spot all day, except when he gets up to eat or to use the bathroom (which is only 3x a day for bathroom--not sure if he has to go more and is just holding it because he knows he can barely walk or not). And it's so hard for him to walk. He has callouses on both front elbows and a growth on his back foot that has broke open and bled on two occasions (the doc said it was a wart, but she barely looked at it). Whenever I am gone from the room or house and come back, he always wags his tail though.

Most recent vet visit was last week. X-rays showed nothing wrong with spine or hips, and the doc said liver was slightly enlarged. I don't think his hind legs were x-rayed, but she did a neurologic test on them and she said it was nothing neurologic. Wants to begin meds for hypothyroid and Vetoryl 60mg. Frisco is 68 pounds. I am terrified to start either one, and am hoping for reassurance I guess.

And that's my story. Now for questions...

1. Based on Frisco's lab results, does anyone feel one problem should be worked out before the other (cushings vs. hypothyroid)? Does it even look as though he has true hypothyroidism, or do you think the results are that way because of the cushings?

2. Because he is in such bad shape right now, should I even give him Vetoryl? I was under the impression that the meds get the whole peeing, drinking, eating and hair loss issues under control (which Frisco doesn't even have, except the eating), but do little for hind leg weakness, which is his biggest, if not only, concern right now. I would absolutely hate to put him on a medication that may make him feel horrible for nothing.

People more knowledgable than I am will be along soon with a lot of questions for you so they can give you the best help.

In the meantime, I will just say that we see too many dogs started at too high a dose of trilostane/Vetoryl.

I think it is better to start with too low a dose than too high. The worst that can happen with too low a dose is that after allowing time for the medicine to reach its full effect (about 30 days) symptoms will not be contolled and an increase will be needed. The worst that can happen with too high a dose is literally death.

Every dog is different, but I would advise starting with no more than 10 mg twice a day. This is where my Rosie started three years ago and where she has remained to this day, and although I can't promise that your dog will be the same, it seems a good safe place to start.

I have only a moment to post which means that this will be short, sorry :(

From his LDDS and ACTH tests and along with abnormal blood work values and symptoms I think Cushing's is a real possibility. Dechra, the makers of Vetoryl, recommend a starting dose of 1 mg per pound of a dog's weight so for his weight of 68 lbs the 60 mg dosage the vet suggests is right in line with what Dechra recommends.

You are right, the meds for Cushing's do not cure it, the goal of treatment is to relieve those bothersome symptoms and get that elevated cortisol down within the therapeutic ranges. Improvement in rear end weakness will take a long while to see, hydrotherapy could help with that. One thing to be aware of is when that high cortisol is brought down it may unmask signs of arthritis but there are supplements/medications that can help with that.

When dogs have Cushing's their thyroid hormone levels can be lower than normal which is known as Euthyroid sick syndrome, treatment usually brings those levels up to more normal ranges.

Hello and welcome. Based on all the things that are normal, considering the neurological, spine check ets. It might actually be worth it to see if the vetroyl will help him with the hind leg weakness.
Some dogs do not regain a lot of muscle, some regain some, and others seem to really make a turn around. You never know.
It is of course your call, but I'm thinking if there is a chance to help him and he might feel better, then maybe it is worth it to try. Not all dogs have every cushing symptom. Mine never had most of them until recently and still doesn't have some.

Hi! I'm not qualified to give advice but I can understand how scared you are to start your fur baby on new medicine, especially since he's not doing well. I hope he's feeling better soon! :o
