View Full Version : Corkster - adrenal tumor/3x daily trilostane (Corky has been released)
Hi Teri,
Hope Corky's stim is good:)
I was just reading on line about the tylan for staining. The consensus was that Angel Eyes has a much smaller amount of tylan in it and that you could get a 10 day supply from your vet of regular tylan for alot less money and because it is a stronger dose, you only need to use it for 10 days.
I wanted to share that with you:) Koko is getting some soft stool here and there from the Angel Eyes. It has to be that because his poops are pretty consistant with no problems otherwise.
Let me know how our Corkster is doing, I know you said you usually have blood work done too so I am hoping everything is really, realy GOOD!!!:D:D:D

Thank you so much for thinking about us and for hoping that everything is good with Corkster's test results.
I have a feeling that his cortisol level is on the rise again. I've been seeing signs that really make me think it is higher. I hope I'm wrong.
I will be giving Corky the Tylan tomorrow when he eats his dinner. I'm thinking positive thoughts that it will help him. Since Corky doesn't get groomed with his short hair, I'll have to wait until he sheds some of the old hair. He doesn't shed very much, so this could be a long time.
It's possible that the Angel's Eyes did cause a change in Koko's stools. Sometimes something different in their diet can cause a change.
I'll keep everyone posted when I get the test results. I'm hoping to have them Wednesday.

We just got back from having Corky's second blood draw. I should get the results either tomorrow or Thursday.
Corky's adrenal tumor has increased in size. The width is about the same, but the length increased from 3.5cm, which was 2 1/2 months ago, to 4cm.

Terri, I've very sorry to know that Corky's tumor has grown. I'm guessing this is not unexpected, but that does not change the fact that you'd rather it was not so. Corky is such a little trooper!! Please give him some big hugs for me, and let us know the results of the testing as soon as you can.

Corky and I both thank you for the hugs. He is a little trooper. I tell him this every day.
Since May, the length of the tumor has increased by 1 cm. It seems like it is growing faster now. I will post the results as soon as I get them. I have a very strong feeling that his cortisol level is higher again, as I've been seeing signs of this recently.

Corky's tumor is growing faster. I went back through his U/S results. I knew down deep in my heart that it was larger.

Hopefully, Corky's blood chem panel will still be good, but I have a very strong feeling, due to signs I've been seeing, that his cortisol level will be up. I'll just have to wait until I get the results.

Dear Terri,
So sorry to hear about the growing of the tumor. I do not know a lot about this disease yet, but I think a well known Veterinary Endocrinologist said that the adrenal glands themselves tend to enlarge with Trilostane. I am sure someone else knows better than me if that is the case. It seems like if that is the case, maybe that would also cause the tumor to grow? How committed are you to trilostane? Because I am less and less.
So sorry this post ends strange but you understand :eek:

Corky's cortisol level actually went down. It's the lowest it's been in a long time.:D:D:D
Pre - .09ug/dl In September it was 1.1
Post - 3.0 The last time it was 7.4
He will be staying on the 40mg of Trilo BID.
His T4 was good. It was 2.1 with the Ref. Range of 1.0-4.0.
Most of the results from his blood work are in the normal range.
GGT - 15 High Ref. Range - 1-12 The last time it was 8.
BUN - 49 High Range - 6-31mg/dL
BUN/Creatinine Ratio - 54 High Range - 4-27
Triglycerides are high, but these always fluctuate with Corky. Sometimes they're high, and sometimes they're in the normal range.
I really feel good about his test results.

Now I know what you have been up to.:) So happy for you and the Corkster. What a wonderful surprise that the numbers were better than you thought.
I keep wondering what Maddie's will be when we go. A part of me think they are up, but she seems so good in many ways. Her coat is so beautiful and soft again. Her eating is great. I think I notice more water drinking, but then I think I just notice when she drinks. The only really change is more panting, but I think it is her coat is so thick and she just might be hot.
It seems like you have been at this a lot longer than just a year. You were there when we started this journey in February--time goes by soooo fast.:D:D See, they are troopers and keep on going like the energizer bunny. Who said they cannot live out their normal life span...:)

I know I still watch everything with Corky. I notice every little thing.
I hope Maddie's numbers will be good. At least if the cortisol level rises, the dosage can be increased.
I still have to chuckle when I think about Corky's IMS comparing him to the energizer bunny.:D Our furbabies are such troopers, and they do keep going.:)

We are feeling very good about this too! Simon is getting ready to do his happy dance for Corky where he rolls over and pushes himself around the bed on his back and then ends it with a bow! I just have to get the music ready. (Actually the music is me singing some crazy song, so I can't do that while I am typing.)
I haven't been on the forums for a while, so I do hope that this good news came after I last talked to you and you were expressing some worries. I hope this means worries are resolved at least for now - hopefully for good!
That Corkster is a trooper - quite the inspirational dog!

Corky and I are going to be taking a short break from Cushings. We need to get away for awhile. There are other issues that need to be taken care of. We won't be gone long, but it's something we need to do.

Corkster and I are back. We are fine. I had a scare with a very close, elderly family member and I thought I was going to have to fly back home. I knew I wasn't going to be able to concentrate on the forum. She is doing much better now.

Hi Kim,
Corky took the Tylan for a week, and then I kept him off of it seven days before his last stim. I then gave it to him for a month. I stopped giving it to him two weeks ago, as I didn't want to keep him on it longer than that. Corky normally doesn't shed very much, unless he's afraid of something, especially when he has to go his vet or his IMS. You should see my car seat after I take him to see them!:D It will probably be a long time before he sheds his discolored fur. I hope it works, too.

I guess it is something we will have to live with:mad:

Thanks for checking on us. Corky is doing good, except for the cyst, if that's what it is on his back, it's getting much larger. It has been drained flat several times, but now it is so much bigger. His IMS doesn't want him to go under anesthesia just for that. I'm making an appointment with his vet, who is also his surgeon, to see if there is a way it can be removed without putting him under. I don't know if he would be able to give him a sedative and then use a local to remove it.
As long as the red/orange color doesn't bother Koko, I wouldn't worry too much about it. I stopped Corky's Tylan after having him on it for a month. I know he has to shed the old fur in order to see if the new fur would be white.
You're absolutely right! There is always something happening with our furbabies.

Sorry to hear our Corky's cyst has grown. Is this something new you just learned of? I guess I too wouldn't feel comfortable putting him under anesthesia. I pray they are able to do a local anesthesia instead. aside from that, I am glad he is doing well otherwise. And I hope you too are just fine. Princess is doing good, but the pyoderma skin issue has not resolved yet and she has been on the antibiotic for almost 2 weeks. I have another round of different antibiotic once she has finished the first one. UGHHHHHHHHHH. yes, as always when it's not one thing it's another. Hi my dear friend. Corky has had the cyst for quite awhile. It never got as big as it is now. It's a little red near his skin. It seems to be getting bigger everyday. I'm going to try to get an appointment with his vet tomorrow.
I hope Princess gets rid of the pyoderma skin issue soon. Hopefully the other antibiotic will work. I hope she's continuing to do well otherwise.

I hope the vet can help with Corky's cyst. The one on Squirt's face that was drained hasn't healed but doesn't fill back up. It just looks crusty all the time, almost like one of her andenomas, so I guess she will be going in to see Dr B soon, too. Hopefully these are nothing major for either of our babies! ;) We can compare notes!

I wanted to try to get him there today, but I need a first morning appointment, or a first appointment after lunch for Corky, as he shakes so badly, pants, and tries to get out the doors. He has an appointment tomorrow morning at 8:45, which is the first morning one. This way Corky won't have to wait to see his vet.
It is always something with our babies.:(

I finally got home with my totally stressed out little boy. The growth is definitely a cyst, and it has to be removed. Before I even had a chance to mention anything about anesthesia, Dr. R said that he really didn't want Corky to have anesthesia. He said he would give him a mild sedative and use a local. Once he sees how Corky does with this, Corky has 4 smaller ones that will be removed if he does well with the local. A big sigh of relief! He will be getting this done next week.
Dr R also checked his teeth and there are no problems with them. He won't be getting his teeth cleaned in January. Unless there is a problem that is observed with Corky's teeth, he probably won't being getting them cleaned anymore.

I am so glad that you have an answer and a plan in place for the Corkster. I KNOW that is a relief to you! :) Using a local and sedative sounds much better than anesthesia and hopefully they can get the big cyst out with no problems at all so the smaller ones can be done with minimal concern.

I am very relieved to know that there is an alternative way to remove Corky's cyst. It's also great knowing that Cork's IMS and vet are totally in sync. It helps make my life a little easier.

I am sorry that our Corky needs to have those stubborn cysts removed but as the others very glad it is a simple procedure for him. I wanted to call you soonest I read your post earlier today but received phone call from school to pick up little one who wasn't feeling too great at school, so had to go for her. Our poor Corky, boy is he building some character in life huh, but them again it would have been better for you had he just been a spoiled, rotten brat and do without all that great character--lol. He will be just fine, as he always has been. Glad he is having a special steak dinner tonight. I knew you would somehow plan that because you are such a loving mommy. hang in there. Hi Sonja,
Thank you for asking about Corky and me. We're doing good. Corky's hind leg weakness definitely improved when he started the Trilo. He is having some problems with his hind legs now, but that is mostly due to his arthritis and the pins in his hind legs. He sometimes falters when he jumps up on the couch and when he is walking, (actually running). You would never know it by the way he takes off runnning for his morning walk.
He has his surgical procedure to remove his large cyst on Tuesday morning. It's going to be difficult to keep him from going for his walks, as he's always ready to go at the crack of dawn.

Time goes so fast, Terri, Thanksgiving is here and gone and the holiday weekend almost gone.

Thank you Lil Sis for your healing thoughts and prayers. I'm soon going to be getting ready to take Cork for a short walk and then take him in for the procedure. Since it's still really dark here, I'm going to have him walk around the parking lot where the vet is, as there are several places that are open, and it is well lit. I definitely will let you know how everything turns out. Corky will definitely want to cuddle and snuggle. He's sleeping next to me right now.

Thank you everyone for your well wishes, thoughts, prayers, hugs, etc.
The surgery itself went well, but when I got Corky home, he started having issues. He started straining and grunting, and when he did this, there were drops of pee. His grunts started getting very vocal. He is now under observation for the rest of the day. Dr. R is giving him a valium, is putting him on an IV catherter and IV fluids. He thinks that Corky had a reaction to one of the materials that was used. He is flushing his system out.
Corky has to be in a cage, which breaks my heart. They put him in a run so it wouldn't be as bad for him.
I'll keep everyone posed as to what is going on.

I don't know what it is about cysts...the dogs seem to get them for no reason as they age...my last 2 Aussies had them and I surgically removed a cyst from each....totally non-cancerous, but bothersome.

Corky's had many cysts removed, but never had a problem before. They didn't put him under this time, but used an asthesia type sedative and a local.
Dr. R said that some breeds are prone to getting cysts, and that a Boston Terrier is one of those breeds. Yuck!

That sounds like good news Terry. I know it stinks for him to be in a caged environment, but they'll keep an eye on him. I wonder if these cysts are a "terrier thing"?? I have friends with terriers (Scotties, Yorkies) and they had problems with cysts and fatty tumors as well.
Just one more thing to worry us!
By the way, Boston Terriers are beginning to grow on me as well....Corky is just so cute!:)

Cysts are definitely a Boston Terrier thing, and I imagine this is probably true of most terriers.

Corky is home!:) He couldn't wait to get out of there. He was back to pulling to get out the door, and he kept jumping up to give me kisses.
The first thing he did when he got in the house was to go right to his water bowl. He is now eating, and he's extremely hungry.
They left the catheter in just in case there would be a problem. They also gave me a copy of all of the information from today in case I would need it, which I hope I won't. The tech even gave me her home phone number if I have any questions.

Corky and I had a good night's sleep. Corky got up twice, once to get a drink and go potty, and the second time was to get a drink. He's still sleeping next to me. :)
He doesn't seem to be in any pain. He gets the catheter removed this morning.

Thank you for checking on Corky. He's been sleeping alot more the last few days. He had diarrhea for a day and a half. It finally eased up today. I gave him pumpkin with one of his meals. I really think that it was due to all of the stress he went through, along with the meds he's on. He was so much better today. The last two days he went for his long walks.

Hi everyone,
I got the results today from Corky's 3 month stim and blood chem panel. His cortisol level is up again. I don't think it will ever be stabilized.
The numbers in parentheses are the results from the tests in Sept.
Cortisol Pre - 1.3 (0.9) Post - 7.3 (3.0)
Even though his post level indicates an increase in his dosage, his IMS is keeping him on the same dosage for three months, as he is having a few issues. If I start seeing any increases in any of the signs, he will go back for a stim test before three months.
Except for a few things, all the rest of Corky's test results were in the normal range. The ones that are not in reference range are listed below.
ALT(SGPT) 127 - HIGH (73)
BUN 45 - HIGH (49)
BUN/Creatinine Ratio 56 (54)
Triglycerides 337 - HIGH (340)

Is Corky acting ok? Those numbers are still within acceptable range as long as he is not showing signs, or an increase in signs. But, of course, you already know that! :rolleyes:
What was Corky's creatinine? I am assuming normal and if so then the high ratio and high BUN aren't all that bothersome at all. The BUN can be effected by many things and as it rises, it throws the ratio off. So the creatinine is what is important.
Keep your chin up, sweetie! You are doing a wonderful job. Adrenal tumors can take a bit more tweaking and sometimes a higher dose to keep controlled so don't get discouraged.

Corky is showing a few signs, but they are not drastic. He wants to eat a little more and is drinking more water. So far it's a small increase, but I am going to start measuring everything again.
Corky's creatinine is good, 0.8, with a range of 0.5-1.6. His creatinine has consistently been within the ref. range.
I've resigned myself to the fact that as his tumor gets bigger, his cortisol level will also increase. It's just something that I'll continue to deal with.

I wouldn't be disappointed with those last stim numbers. They are still acceptable.
I don't know if this will help but I have blood tests done every month and it is surprising how different the numbers are on some of the results. Different figures go up and down and my specialist said that is normal. Our bodies are pretty complex and he said he never reacts on just one test because it takes several to see a trend and then he will change or adjust medications.
I think that while Corky's numbers are still within range and unless any of the symptoms cause problems then not making changes is the best answer.
I do remember that I felt I needed to be constantly doing something so that I could help Buddy but sometimes (felt like always) you just need to wait it out.

I'm not really concerned with the highs in Corky's blood work, as these have been fluctuating. His dosage has been increased before when his level was lower than 7.3. My main concern is that it went up 4.3 points in the last three months, after dropping 4.4 points after 30 days of being on 40mg daily BID. I trust my IMS and her judgement, and she didn't want to increase his dosage while there were other issues. I will take it one day at a time and continue to monitor Corky's food and water intake.

I am sorry about the rough two weeks for our Corkster. He is such a trooper, I know this week will be better!!!!!

Addy, thank you for your very kind words my dear friend. Our Corkster definitely is a trooper. One of the issues he was having is improving. The last few days have been much better. I did cheat a little and gave him some bread. Actually, after everything that he went through, he had a hamburger on a roll. He hadn't had one in such a long time. He really devoured the whole thing. :D He loved it. It was a special dinner for him. Love you, too.
I understand what both of you are saying about your hair. I hate to say this, but when I made that comment, I was referring to the kind of day I was having. I need to watch how I phrase things.

Those bully sticks are great! Crys used to LOVE them! Of course, they didn't last very long with her jaws and enthusiasm but she sure enjoyed them while they lasted. She would hide the pieces all over the house and then walk around whining for a bit, go get all the pieces, mouth them up til they were good and slimy again, then re-hide and start walking and whining all over. She shoved slimy bully sticks under my legs many, many times as I sat and studied. :D She tried to be good and not eat them all at one time, but she just couldn't resist! :p They kept her occupied and stimulated for hours, which was a miracle with the Demon Dog! :D
I bet Zoe will love them, too!

Thanks for checking on us. We're doing good. Corky's stools finally have form to them. They're still soft, but they've been that way for almost 3 years. His mouth still quivers and his teeth still chatter in the morning, but it doesn't seem to last as long. He had a mini episode with the straining about two weeks ago, but nothing since.
He's not getting a burger for Christmas, but he is getting ham and turkey with all of the fixings.
I hope Zoe enjoys her bully stick, even though I have no idea what it is. I agree with you and Sonja. We have to give them what they really like.
I hope you and your family, and of course this includes Zoe, have a wonderful Christmas.

I wanted to share this with everyone. I have been in contact with Dr. Tim Allen from Dechra this week. I had a question I wanted clarification on. While we were corresponding, I told him that the only white fur on Corky that was discolored was where I had used the KetoChlor. He did tell me that the pink discoloration was due to the Ketoconazole and should fade over time.
I also asked him if there was any research being done on three times daily dosing with Trilostane. He did tell me that there are some dogs that require dosing three times a day.

Finally you know what Corkey's discoloration is from:)
I emailed him last week too to ask about Zoe and he was so nice, replying really quickly. I also was touched that he refered to her by name in his email back to me. Something small, but it made an impression, instead of not refering to her or calling her "your dog".:)
He told me if I start Zoe on low dose trilostane do 1 mg per kg, I could do it once a day or split the dose for twice a day and that in Zoe's case I should round down. Now I have to review his info and Dr. Oliver's with IMS tomorrow.
Were you thinking about three times a day dosing or just inquiring?
How is our little boy? I loved his Christmas pictures!!!!!

I was wondering about the three times daily dosing, as Corky has been very hyper in the A.M., before he has his Trilo. In the evening before he gets his dose, he's also hyper. After he eats his dinner, he starts looking for biscuits and more treats about 2 hours after he eats. When I take him for his next stim test, I'm going to discuss this with his IMS. I'm glad you liked his new pictures. I added 2 of him wearing his jacket.

Thank you for checking on us. Corky is going in for a stim test, a blood chem panel, and an U/S on Tues. It's been six weeks since his last stim, and his symptoms are getting worse. He's really been hyper, is now always looking for more food, and is drinking more water. His belly is hard again.
I did call his IMS about the 3x daily dosing. We're going to discuss this on Tues. There is a member on CCAC who has started using 3x daily dosing. I spoke with her on Sun. It has been helping with the symptoms.
I haven't been posting much lately, but I have been following Zoe's thread, along with a few others. I'm just washed out.
I'm hoping you get the adrenal panel results soon. Then you'll be able to start the proper treatment for Zoe.

Sorry to hear Corkey's symptoms are getting worse. I don't blame you for being washed out. The research and thinking takes alot out of a person. You do so great with him. I keep reading about the doses needing so much tweaking, hopefully, that is what our Corkster needs. He is such a trooper through his ups and downs.

I have been searching and searching this site to find a scientific research report that I once translated for this site. But...I can't find it. Perhaps it was from before the site "crashed" and was lost.
I will translate a portion of that report for you, you may find it interesting :) It's by 2 veterinary endocrinologists from the Unversity of Gent, Belgium, written in 2004.
"Options for the treatment of canine and feline hypercortisolism.
It's our experience that Trilostane is better tolerated and showing less side effects when dosing lower then advised (1 mg/kg per day). Trilostane is short acting: 2 till 6 hours. This means there's a relative short period of cortisol supppression but in most cases once a day dosing is sufficient to improve the clinical symptoms. However, when clinical symptoms such as poly urie, polydipsie and increased appetite are not sufficiently suppressed, twice and sometimes even 3 times a day dosing can be helpful in reducing the clinical symptoms."
As you see, 3 times a day dosing is being used here in cases where the clinical symptoms aren't suppressed enough. And that was already 7 years ago :)

I just got a call from Corky's IMS. His pre stim was 2.5, and the post stim was 10.7. :(:( In the last 6 weeks, his cortisol level went up from 7.3 to 10.7. :(:( She is going to be talking with Dr. Allen to see if we are just going to increase his dosage, or if we're going to try the 3x daily dosing.
In the meantime, I'm playing the devil's advocate. I'm going to give him 25mg tomorrow morning and 20mg tomorrow evening. He is constantly looking for food, and drinking much more water. He just finally settled down after I gave him his Trilo this evening. I gave it to him an hour earlier.
There are a few things that are elevated on his other blood work, but I won't have a copy of the results until tomorrow morning. When I get them, I will post them.
On a positive note, his tumor has not increased in size in the last 4 1/2 months.

Oh no! Sorry his levels have gone up. Here we go again huh? Ugh :(:(. Nothing that a little dose increase can't fix right or maybe even better the 3xs per day theory. I hope whichever path you take it resolves his symptoms quickly so he can feel comfortable again on all levels. Oh and that is great news about his adrenal tumor. He got up 3 times during the night, but each time he got up, he went right back to sleep. He has to be exhaused from being so hyper.

I'm still waiting to hear what we are going to be doing. I did give Corky 25mg this morning, and it has made a big difference.
I have the results from his blood work. His BUN and BUN/Creatinine levels have decreased a little, even though they are slightly above the normal reference range. His BUN is in the normal range. His Triglycerides are high - 860mg/dl, with a ref. range of 29-291. This could be due to the extra treats and biscuits I've been giving him, since I don't want him to feel like he is starving. The only other test result that was elevated was the platelet count, which was 461, with a ref. range of 17-400. This has fluctuated over many years.

Thank God about the tumor not getting larger. I am so sorry to hear Corky is going through the roller coaster ride again. I am thinking of putting Apollo back on the Trilostane slowly but now I am told he has intestinal problems and started once a week B12 shots. Will email DR. Allen at Decha to check if okay to start. How is Corky's hind legs doing. Apollo has problem with all four legs now.

Corky's hind legs are doing good. He's been losing his footing, but that's because he's been so hyper and in such a hurry to go for his walk, or should I say run. He was better today since I gave him 25mg this morning. I hope to get an answer soon as to what treatment plan we are going to be using.

I am comfortable with this plan. The 20mg 3x daily dosing scared me, as that was a 20mg a day increase. That's why I asked if I could do the 20, 20, and 11, as I have plenty of both of the mg dosages. I hope this really works for Corky. This is day number one.

Thank you Marlene. Corky is much calmer today. He's actually sleeping. I thought I might have a problem waking him up at 4:30 A.M., but once I did, I gave him his Trilo dose wrapped in some turkey breast. He took it and went right back to sleep. He only woke up once in the middle of the night to go potty. So far, so good.

I'm just wondering why you're dosing at 4:30am? Is this to fit around your personal schedule?
Also, and this one goes out to The Forum: I'm thinking of how Trilostane is better absorbed with food. Does anyone know how little food is required. Is wrapping the capsule enough, or is a light/moderate/full meal needed? I've always dosed with a moderate to full meal myself.

I give him a dose at 4:30 A.M., as the other 2 doses are given at 12:30 and 8:30 P.M. I found that these are the best times for giving Corky his Trilo every 8 hours.
I don't really know how much food they have to have with the Trilo. I don't remember seeing any information on the amount of food to give them. When Corky was on BID dosing, one dose was given with a meal and the other one was wrapped in a slice of ham or turkey breast. Now he gets 1 dose with a meal and the other 2 with his ham or turkey.

Thank you for asking about our Corkster. He's been on the 3x daily dosing for 4 1/2 days now, and isn't having any problems with it. He finally slept all night last night. His water intake has decreased. He's almost back to his normal amount. He actually left a little bit of food on his plate today. If I have to wake him up to give him his Trilo, he takes it and then goes right back to sleep. I'm hoping that this is what Corky needs.
