View Full Version : Maggie - 10 yo Dachshund on Trilostane

Hello, and I'm hoping you can give some advice/reassurance. I've spoken of my male Dachshund Joey on here before, who had a bilateral adrenalectomy, but this is about his littermate sister, Maggie, who has pituitary dependent Cushings.
She weighs 20 pounds. We started her on 30 mg Vetoryl/day and were doing OK, but then her levels came back very high, so we doubled her dose to 30 mg every 12 hours. She also had a pancreatitis attack before her dose was doubled, and we nearly lost her.
It's been about 6 weeks since her pancreatitis attack, and she is doing great and tolerating 30 mg Q12H well. She runs around like a puppy, which she hasn't done in some time.
Just got new ACTH stim test results today and they were 2.2 pre and 9.6 post. The doc wants to just slightly increase her dose by 10 mg/day. His goal is to get her post test < 7.6. I see in a study on twice daily trilostane dosing that this is a goal in that study, but everywhere else I read, it's 5.5? What are your thoughts on this?
I know her dose is possibly getting kind of high - it will be 70 mg/day - what are some max mg/kg doses that you see on dogs here? At what point should I be worried that her dose is too high? Is 10 mg increase aggressive enough based on her levels?
I work 10-6:30, so I give her doses with meals usually at 9 AM and 7 PM (I've tried to give her evening dose at a more appropriate interval, but then I kept forgetting it, or giving it to her at 2 AM in the middle of the night when I would remember). So, I am going to give her 30 mg at 9 AM and 40 mg at 7 PM (where there is a 2-hour longer interval).

Hi Carson,

Corky weighs 18.3 pounds now, and he is currently taking 60mg of Trilo, TID. He started at 20mg once a day, went to BID, and now TID. I know they say that the upper end of the normal reference range should be 5.5 when using Trilo, but there are some fur babies that can do well on cortisol levels that are slightly higher.

When Corky's level was up to 10.7, we increased his dosage by 11mg, which lowered his level to 9.2. We then increased it by 9mg, and his level was 4.0 on his last stim. A 10mg increase might be enough to get the cortisol level in range.

I hope Joey is doing well.

Hi and welcome from me as well!

We have seen some dog's cortisol levels drop while being treated with the same dosage over a period of time. Also how a dog's feels and if the clinical signs are controlled are just as important as the ACTH stim numbers.

My Apollo is also on Trilostane. He weighs 10lbs and is on 10mg. for now but will probably have to increase soon. Read the recommendations from Dechra. When you do increase it should be a small percentage. Each dog is different.
