My dog has diabetes and cushings. She is a 13 pound Yorkie (was 10 pounds before the weight gain from cushings). She is on 40 mg of trilostane and 14 units of Insulin (i'd like her to be on a higher dose). Lately she has gotten extremely hungry at night and aggresive about it and begging and whining and starts licking things (like my bed). Does anyones dog do this and how do I handle this?

Judging from the size of her insulin dose compared to her weight, I'm wondering how well her Cushing's is controlled.

Has she had a stim test recently to measure her cortisol levels?

How are her blood sugar levels?

Poorly controlled, both diabetes and Cushing's can make them really really hungry.

There are dogs who just need more than the average amount of insulin and it's not a problem as long as their blood sugar is good. If it's not, then they would be labeled as insulin resistant and in this case the Cushing's not being fully controlled would be the likely reason.

Do you test her blood sugar at home?

I invite you the the sister forum to this one for dogs with diabetes:

The main website, which talks about diabetes in general, is at www.k9diabetes.com (http://www.k9diabetes.com).

Boy, this is a tough combo to manage; I've dealt with one or the other, but not both at the same time.

The late night hunger is a typical Cushings indicator, I dealt with a lot of 2:30am barking for milk bones at one point, which led me to suspect Cushings...and I was right on that. But I'd say get the blood sugar levels right first. That's actually much more critical short term. .

Then work on the Cushings issue; the trilo, if it is dosed right and your Yorkie responds, should bring down the hunger pangs. If not, maybe a new dosage level is needed (or divided into twice daily dosing)...or another med may be tried.

Welcome
We would like to know what test were done, which cushing your Yorkie has.
Apollo is 10lbs and is only on 10mg now 12.5mg. The dosage seems very high to start.
Below is weight and starting dosage.
Table 1: Starting dose
Weight range (pounds) Weight range (kg) Starting dose (mg) ONCE DAILY
>3.8 to <10 >1.7 to <4.5 10
>10 to <22 >4.5 to <10 30
>22 to <44 >10 to <20 60
> 4 to <88 >20 to <40 120 (2 x 60 mg)
>88 to <132* >40 to <60* 180 (3 x 60 mg)

Ask away.
